Maintain Fully Licensed IPM Accreditation Status<\/span><\/h3>\n\n\n

This\u00a0 page is for those golf course facilities that have met all of the requirements for Fully Licensed IPM Accreditation status and are submitting their Full Desk Audit to maintain this status.\u00a0\u00a0 The information required is exactly the same as what has been submitted in previous years.<\/p>\n

Note that all forms and the registration fee must be submitted before the Auditor will review your Full Desk Audit.<\/h4>\n

PAYMENT<\/h2>\n

Reminder that the registration fee is $425.00 for forms and payment received prior to January 31 and $500.00 if received after this date.<\/span><\/span><\/p>\n

Payment can be made either by Cheque or Credit Card. Cheques payable to Plant Health Atlantic are to be mailed with completed Full Desk Audit to the address below.
Credit Card information can be included on the form or contact Plant Health Atlantic office by telephone 1-506-440-9656 <\/strong>.<\/span><\/p>\n

FULL DESK AUDIT FORMS – Forms are now available in two formats<\/h3>\n

DIRECT COMPUTER EDITABLE FORMAT:<\/span><\/h2>\n

As of April 2015 the Full Desk Audit forms are available in a direct entry format.\u00a0 The requested information can now be entered directly on the forms and submitted as an attachment to an e-mail.<\/p>\n

NOTE:<\/strong>\u00a0 “Editable PDF” Full Desk Audit document allowing you to complete and submit the forms
electronically.\u00a0 It is imperative that you follow the steps as follows:<\/p>\n

1)\u00a0\u00a0\u00a0 Save the document to your computer
2)\u00a0\u00a0\u00a0 Enter the required information in the designated fields\u00a0 (you do not
need to complete the entire document in one sitting however, always save
your work prior to exiting)
3)\u00a0\u00a0\u00a0 Once the document is complete, save and email<\/a> to Plant Health
Atlantic<\/p>\n

Alternatively, the document can be printed and completed by hand.<\/p>\n
